The front door may be locked after 4:30 and on saturdays. Please text me when you arrive for an appointment around these times. 

 You may need an annual doctors requisition, depending on your insurance requirements. Please be prepared to pay upfront in full and submit to your insurance yourself if needed.

 

NEW: No Show Policy

If you no show for an appointment there will be a fee charged of $25.00 on to your account that will need to be paid before your next appointment.

 

NOW AVAILABLE: Emergency appointments

If you are in need of an emergency appointment which requires me to fit you in outside of my normal schedule, you now have an option to request an SOS appointment with an additional fee of $25.00
